HSA sent payment voucher to an unknown fax number not associated with healthcare provider, no company involved knows why. How to handle / anyone else to contact to ensure funds are recovered?

Hello. A family member recently had a routine in-network doctors visit that resulted in a patient balance for us to pay. The claim was submitted to our insurance company by the provider as usual and we initiated a payment via the HSA's website as we always have, which showed up as cleared and released to the provider several months ago. We're now receiving bills for that visit that are about to go past due. The HSA forwarded a proof of payment to the provider's billing company to clear things up, but according to them, while the doctor and merchant name are correct, the fax number where it was originally sent is not theirs (google comes up with an unrelated medical lab with no affiliation to this provider or any of our claims). Whoever received the original fax with the payment voucher processed it and the funds have long since cleared.

Nobody that we've spoken to knows how or why it was sent to an unknown fax number. An HSA rep said that they receive billing information from the insurance company, the insurance company said it must be wrong in Blue Cross Blue Shield's system, the billing company says its right in BCBS's system and they've never seen that fax number before and don't know how the insurance company got it, and the provider says everything's handled by the billing company.

Having said all that, the HSA company has initiated a fraud investigation, but it could take up to 45 days and none of the reps we spoke to had ever heard of this happening with an ACH transfer. My question is, are there regulations that protect these electronic transfers, and/or are there any additional agencies we should contact to ensure these funds are recovered? The HSA reps we've spoken to recommended paying the bill out of pocket to avoid it going to collections while they investigate, but the insurance company rep said don't pay out of pocket because it's not a legal bill (based on the proof of payment), while the billing company rep said if we don't pay it will go to collections. Any insider advice on how to handle would be greatly appreciated. GA, USA.

[Pennsylvania] Adding 16 year old driver to insurance

    Posted: 03 Sep 2020 03:11 AM PDT

My son has his permit and is a month away from getting his driver's license (which I don't think he needs btw). I am debating on whether to get a second car with basic coverage rather than adding him to the leased car with full coverage since I think the difference might be significant. However, my insurance said that he would need to be added to that first vehicle regardless if he drives it since he is in the household. Does that sound correct?

How to take behind the wheel test without car insurance?

    Posted: 02 Sep 2020 03:20 PM PDT

Hello I live in California. I've already passed the written exam and I just made an appointment to take the behind the wheel test. It's asking for car insurance. However I don't have it yet as my husband's insurance company won't add me without an active valid DL. What should we do? If I use his car for the exam can he show his own insurance? Can anybody else with a car and valid DL do this for me? What should I do? I've never had a DL before in my life. Please help. I just want to learn how to drive.

Dispute with insurance company and previous homeowner while trying to file a claim.

    Posted: 02 Sep 2020 06:38 PM PDT

I was told to post this here, if its not the right spot I'll remove it.

Forgive me this is going to be long. My wife and I just bought our first house in 2019, we live in Colorado where yearly hail is a regular occurrence. In 2018 there was a series of nasty hail storms that destroyed a fair amount of property (cars, roofs, windows, etc). The previous homeowner said he had replaced doors, windows and the roof recently because of hail. Everything looked new, everything checked out. We had our home inspected by a 3rd party inspector who did a 4-point plus a little additional stuff for us. There was no evidence of visible hail damage on the roof, per the inspector. Everything checked out, we bought the home.

Fast forward to 2020, it just hailed and we are attempting to file a claim through our homeowners insurance to fix our roof and they are kicking it back because of pre-existing damage. Apparently the previous homeowner filed a claim, pocketed the cash, and never actually fixed the roof. The insurance adjuster says they have pictures from then and now, and the damage lines up perfectly enough for them to outright deny the claim.

I'm very confused on what to do here. Fixing a roof is thousands of dollars and we bought coverage when we moved here with a low deductible expecting to have to pay out for hail damage and now its getting kicked back because of a previous policy with a previous homeowner? The adjuster is telling me it is a civil matter but between who? Me and the previous homeowner? Me and the inspector?

The previous homeowner knowingly made the claim and sold us the house in the current condition preventing us from filing a claim now. I'm not sure what to do here or who to contact to get this corrected. The previous homeowner has moved and since changed phone numbers, so getting in contact with him to talk about this has been rather difficult.

Car rear-ended, but drives okay. Auto shop calling it a total loss?

    Posted: 02 Sep 2020 11:45 PM PDT

    Hi,

I bought my first car last week (used 2014 Toyota Corolla with 80k miles, paid in full) in California, with all inspections done and a recent oil change as well. I then took it from SF to LA. Unfortunately today in LA, someone rear-ended me at a major intersection, and we exchanged information and I filed a claim with both Allstate (my insurance) and his (Progressive). He agrees that he is at fault.

I took the car to an auto-body shop shortly after, and the guy looked underneath the car for about a minute and told me that most likely this was going to be a total loss, which surprised me. The KBB valuation of this car is around 9-10k, and even with a CCC One valuation, I don't see it going under 7k. He insisted that it was a total loss, and told me that he had friends working in various places and that he could help me 1) get my car classified as total loss because his friend works for an insurance company, and 2) it was definitely a total loss. If it is a total loss, that's fine by me. The way it sounded though seemed a little suspect.

The car works fine -- tail lights, rear-view mirror, etc. No clear issues with the way it drives either. I am going to go for a second opinion tomorrow, as well as call Progressive/Allstate adjustors to evaluate the damage. Here are some photos of the car back as well as underneath:

    https://imgur.com/a/pcCYaqb

I'm wondering if anyone here could say something more meaningful about it just by these pictures, so I know what to ask and expect tomorrow when I bring it into another shop.

My main question is the following: Why was the auto-body shop guy so insistent that this was a total loss? Does he have something to gain from it, e.g. he buys the car for cheap and flips it?

    Home insurance type

    Posted: 02 Sep 2020 10:16 PM PDT

My grandma is looking to downsize and move into a smaller single story apartment. She doesn't want to sell the home yet because of the market but has asked me to move in until such time she is ready to sell. I will be covering all the utilities and paying no rent. I am aware I would still need to get a rental insurance policy for myself to cover my belongings. However would she have to change anything to her home insurance policy since this wouldn't be her primary residence? Just trying to help get some ducks in a row. TIA

(Texas) Renter's Insurance Coverage Question, regarding flooded apartment due a burst pipe from drilling.

    Posted: 03 Sep 2020 01:26 AM PDT

I have renter's insurance for my apartment with USAA in Texas. Last night I was mounting a small TV using a stud finder, I found a stud and drilled accordingly. Except it wasn't a stud but the water pipe leading to our fire sprinkler system. Long story short the apartment is now flooded, were looking at a decent bit of damaged furniture, and the apartment itself needs new floors, baseboards, and some walls. We obviously have to also stay in a hotel for the next week or so...

My question is, is this a covered event? It seems like it would be, I already opened the claim... but was wondering since it isn't a natural flood. It wasn't intentional or a mistake, just kinda a fluke accident. I'm allowed to hang things up and put nails in the wall, it just happened to be wrong place apparently... Please let me know what you think, thanks for the help!

Car insurance help, I think I scratched a parked FedEx truck with my car? Wondering whats the worst case scenario if they find out? California]

    Posted: 02 Sep 2020 09:01 PM PDT

I drove home from work tired as hell. On my street there was a large fedex truck parked almost in the middle of the street, with a small place left to pass, I saw a compact car pass by before I went, adn my dumbass tried to go too even though I know my car is a small suv, so it's bigger than that car duh -_-.

The FedEx truck was parked with the emergency lights flickering so I assume the driver was not in the car, so as I was slowly driving by the truck I heard a little scratching noise and freaked out, and just drove my car like 10 feet home into my driveway and then I saw a bit of my car side mirror scratched, like the paint came off a little bit, not sure if it scratched the truck too. My car is white and so was the truck. I'm scared they have cameras or something and can see my license plate and file some insurance claim against me :/ Idk im paranoid.

    Charges for Unsolicited Doctor’s Call?

    Posted: 02 Sep 2020 10:19 AM PDT

Saw a $150 claim that was processed in August for service rendered in July. After contacting clinic, they said it was from when the doctor and I "discussed my xxxx".

The statement from the clinic is false - the doctor called me (I did not call him) and the discussion was "I'm following up to see if you went to see X" to which I responded no and we hung up.

Insurance is going to look into it, but this isn't a legitimate claim, is it? Should I just not answer phone calls from the doctor to avoid surprise bills? The phone call had no substance and I had asked no questions, and it lasted at most, 30 seconds.

Details: 29F, no health issues, NYC, UHC

EDIT: insurance code used was "interactive audio and/or video" and I had called the clinic again who seemed to play dumb say "oh, you didn't talk to the doctor?"

Not sure if the clinic is desperate for income due to being temporarily closed during covid, but this just doesn't sit well with me. I've only been to this doctor twice in January 2020, and those billing issues still haven't been sorted out. They've been using the wrong code, which ends up charging the patient. Third strike and this doesn't seem like an accident to me. Needless to say, I won't be returning.

Hello I'm curious to know why my car insurance is quoted in New York City at $900 a month. I've never before been in an accident and I'm 27 years old with a license for over 3 years. How is it possibly that high ? Maybe there're a reason ? Can I fix it or get lower rates ? I need answers.

    Auto insurance terminating policy?

    Posted: 02 Sep 2020 09:22 PM PDT

Hello guys I was contacted by my auto insurance company and I couldn't pick up the phone. They left a message saying my policy is to be terminated at a certain date. And MAY be reinstated if I provide the following. ( it's the MAY that's worrying me )

Main driver, Proof of residency, Location of garage/parking of vehicle, And a copy of title/registration with owners name or co-owner

Does this mean they are leaning towards terminating my policy? Do they not believe me ? Should I be worried ?

There is 3 insured people on the vehicle one is under 25 (24) and it's a sport vehicle. I am trying to help build his credit it so he is a co owner on the loan. He is my son in law. The car is only for pleasure and hasn't been used much since we purchased it. It's been garaged getting some work done to it.

Wanted to see if anyone has had any Past experiences. And if this means they pretty much don't want my budiness for x reason. From what I've been googling in articles it seems like they are very strict and are quick to Terminate policies. Thank you

Keep the payment and finance the repair

    Posted: 02 Sep 2020 08:38 PM PDT

My homeowner's insurance is covering some repairs to my siding after a storm. They paid the money out directly to me. I was wondering if financing the repair and using that money to cover some high interest debt was an option. Assuming the rates for the financing are lower than my debt ofc. Going to be a while before anything gets done. Not going to do anything rash. It would just be potentially very lucrative for my journey to eliminate my debt.

    Roadside Assistance: AAA vs. Progressive

    Posted: 02 Sep 2020 01:00 PM PDT

    Hi guys,

I've looked at similar postings from the past, but it's outdated and pre-pandemic. Hoping you can speak from your experience inside the pandemic and provide suggestions...

I've been using Progressive for auto insurance and used AAA for roadside assistance.

For the last two-three times I've requested AAA roadside assistance (inside the pandemic), they have been pretty crappy.

    For example:

    1. Car engine would not start at all. In total, I was stranded at a garage for 7-9 hours. 2 - 4 trucks, all spewing the same about how there's no way they can tow my car from the garage under an apartment complex. In conclusion, the last guy said the same shit, but I stressed how long I was waiting, he agreed and helped me roll my car upwards the slope after which he was tipped $20.
    2. Another time, they sent one-two vans in a heated afternoon, saying that these particular vehicles don't tow, so had to wait in total for about 4 hours for a big ol' tow truck to finally arrive.

    Thank you in advance!

    Edit: I live in Florida.

    At fault driver not cooperating

    Posted: 02 Sep 2020 04:43 PM PDT

I was in a hit and run rear end where the other driver was at fault about a month ago. I got a picture of his plates before he sped off, and called the police. I explained to the police what happened and showed them the picture, and they found the guy, and he admitted fault, and a police report was made. The damage to my car is contained to one area, and it is cosmetic, however estimates at the collision center for my dealer say it's totalled. I filed a claim against his insurance when I got a hold of the police report, and explained all of what happened to a tee to the adjuster, she was very pleasant to deal with. She had reminded me though that the driver was not answering his calls and that she was waiting for the police report to come in. 2 weeks pass and I call to see if there are any updates or whether I need to submit more information. I got a letter in the mail 2 days later from the adjuster saying that the investigation is being prolonged because the at fault driver is still not cooperating.

My question is, how long can this drag out if the POS who hit me won't answer calls?

    For reference I'm in OK.

Got hired for American Income Life, but now I’m seeing a lot about it being a pyramid scheme. Is it really that bad?

    Posted: 02 Sep 2020 04:18 PM PDT

I went through a pretty quick hiring process for AIL over the past week. They've fronted half the cost of an insurance license in my state(FL), and expect me to be licensed by the time I start. I'm supposed to start with them once my two weeks with my current company is done. Everything seemed generally okay with them, albeit fast, but I brushed it off because the recruiter said they were urgently hiring for a new office they were opening.

Now, I'm seeing a lot of posts about AIL being a MLM/Pyramid Scheme. I'm already in my final two weeks for my current company, and I've already put up time and money for the insurance license. I'm not sure if I should cut my losses now and simply write it off as a bad job offer where I lost out on $100, stay long enough to get licensed, or see where it takes me. Is AIL really that bad of a company?

    Total Loss valuation too low

    Posted: 02 Sep 2020 07:56 PM PDT

    Location: California

So, I have been in a collision. Not at fault. The car (Tesla Model 3) is considered a total loss. I am going through my own insurance as the other party is underinsured.

My problem now is that my insurance is offering a valuation that I think is way too low. (I know what they say - insured always think their car is worth more than it is...) I am not so much looking at finding an arbitrary value of what it is worth. I am simply looking at what it's gonna cost to find a replacement vehicle that is in fact equivalent.

Here is what I think my car should be worth: ballpark $50k plus taxes and fees

This is the adjusters first shot at it: $37.5k plus taxes and DMV minus deductible

The adjuster didn't even discuss vehicle equipment with me prior to that. I responded with a 6 page letter outlining all the shortcomings. In essence, he did not consider Enhanced Auto Pilot package, Full Self Driving Package, Window Tint, Chrome delete and various other tweaks to the car.

Today the adjuster came back with: $45.5k plus taxes and DMV minus deductible

Still not close to what it costs to get a replacement with the same parameters.

The valuation is done by CCC. I did my research on CCC and believe to have a reasonable understanding how they approach valuations (so that they can claim to save insurance companies 10-15% in total loss claim payouts).

    1. My car is rated "average", which is the 2nd lowest grade. I obviously disagree. Car is just over 2 years old w/ 22k miles. No damage prior to the accident.
    2. CCC uses 2 comps from 400-500 miles away from my zip. Based on my research, that is invalid. there is no shortage of comp vehicles in my area.
    3. I found the listing of one of the comps (the other one is sold). CCC adjusts my value by -$1200 for 19 in sport wheels. the photos of the listing clearly shows it has 18 in wheels (just like mine).
    4. they chose comps that have particularly low milage so they can deduct the most for mileage adjustment.
    5. both comps are lacking Enhanced Auto Pilot, FSD, Tint and Chrome delete (upgrading to FSD after buying one of those comps would be $8k directly to Tesla)
    6. instead of adjusting the cost of the comps by adding everything they are lacking (FSD, Tint etc.), the adjuster adds $6.4k for FSD to the avg. adjusted cost of the "not-really-comps". I understand that they're trying to depreciate the value, but the goal should be not to determine how much to refund me for FSD, but how much it is to replace it. and that is $8k. Tesla in fact increases that number every now and then.

I did my own calculation based on the one comp that's still for sale, and another comp from the same dealership (which in fact is a much closer match). even the VIN of that car is less than 100 higher than my totaled car's VIN (which means it was built on pretty much the same day as mine).

I basically subtracted the mileage adjustment (26c per mile) where applicable. The 2nd comp I use has a higher milage, so I am adding 26c per mile. I am adding $8k to upgrade to FSD. That takes me to about $49k. Now, if I were to purchase one of those cars, they wouldn't have window tint, chrome delete, not any of the other enhancements I did to my car. for the tint I have an invoice. the rest (chrome delete, patches of PPF, interior vinyl applications) I did myself, so no invoice for that.

So, where is the flaw in my logic to approaching the calculation like that? How should I proceed?

    Thanks for helping

Do Kaiser's health insurance plans cover injuries sustained in a car accident?

    Posted: 02 Sep 2020 01:41 PM PDT

[California] I'm looking at buying a gold or platinum plan through Kaiser, and am also in the process of buying car insurance. When buying the car insurance, there is the option to add coverage called "medical payments" which covers any of your injuries if you are in an accident.

Seeing that I will have health insurance, does it make sense to also have the "medical payment" coverage? Unless, of course, Kaiser doesn't cover auto accidents

    State Farm Claim Timeline

Can anyone tell me the typical timeline (number of days) for State Farm to send an insurance adjuster out to assess a home? What about once the adjuster comes, how many more days until they give you a dollar amount for what they will cover? I'm in a predicament having to reschedule home repair appts while I wait, but there is urgency to get this done. My MIL filed the claim bc technically she owns the home and so I am at her mercy... And she has not given me much information - so I'm just left in the dark until she relays the information. Any experience with State Farm or insider knowledge on the timeline is helpful!

How much homeowners insurance should I get?

    Posted: 02 Sep 2020 07:20 PM PDT

    Everything is in CDN $

Condo bought for 250k 2 bedrooms 1 bathroom

6 units total in the building, I'm on the top floor

Q1: how much should I insure globally? (If worst case it goes up in flames and I have to rebuild everything from scratch)

Q2: how much should I get for personal property insurance?

I estimated 50k but I am really unsure. I have a new fridge, range, dishwasher, washer+dryer

Other furniture you find in homes plus TV and PC's etc.

One website says estimate about 10k per room so that's where I got the 50k from.

    Thanks

    Homeowner’s insurance claim report

    Posted: 02 Sep 2020 06:47 PM PDT

I recently had to evacuate my home in CA due to the wildfires, so called my homeowner insurance provide (State Farm), asking if they would cover room and board accommodations such as covering the hotel room cost. I simply wanted to ask this question, IF I needed to stay at a hotel. Luckily, I was able to stay at a friend's place during this period of evacuation, so I did not need to ask for accommodations.

Cut to a few days later, I received an email stating that I had filed a claim(?) and I was confused because I didn't make any mention of a claim, no damage to my house, or anything like that. So I called them back, asking if they could remove this claim, because I did not make this claim. They updated the status, stating that I did not pursue any action. However, that claim report will still be on file. I'm pretty fucking frustrated because I have no idea what effect this will have on my HO insurance in the future, if I will even be able to sell the house etc. Does anyone have any insight on what could potentially happen?
