    SECAUCUS, N.J., June 03, 2019 -- American Medical Collection Agency (AMCA), a billing collections service provider, has informed Quest Diagnostics that an unauthorized user had access to AMCA's system containing personal information AMCA received from various entities, including from Quest. AMCA provides billing collections services to Optum360, which in turn is a Quest contractor. Quest and Optum360 are working with forensic experts to investigate the matter.

    AMCA first notified Quest and Optum360 on May 14, 2019 of potential unauthorized activity on AMCA's web payment page. On May 31, 2019, AMCA notified Quest and Optum360 that the data on AMCA's affected system included information regarding approximately 11.9 million Quest patients. AMCA believes this information includes personal information, including certain financial data, Social Security numbers, and medical information, but not laboratory test results.

    AMCA has not yet provided Quest or Optum360 detailed or complete information about the AMCA data security incident, including which information of which individuals may have been affected. And Quest has not been able to verify the accuracy of the information received from AMCA.

    Quest is taking this matter very seriously and is committed to the privacy and security of our patients' personal information. Since learning of the AMCA data security incident, we have suspended sending collection requests to AMCA.

    Quest will be working with Optum360 to ensure that Quest patients are appropriately notified consistent with the law.

    We are committed to keeping our patients, health care providers, and all relevant parties informed as we learn more.

    Dad passed recently and his life insurance is claiming his policy is lapsed due to non-payment

    My dad had a life insurance plan through Axa and has had it since '93. He passed in April and my brother and I being the beneficiaries filed our claim submitting all the appropriate documentation. They just came back stating that the policy has lapsed, and therefore worth $0. It lapsed in March as they are on a quarterly payment cycle. The problem is that he was too sick at this point. He was barely awake for an hour at a time and was heavily medicated during this time.

    This money would be a lifesaver for both my brother and I and I am just at a lost at how to proceed here. Does anyone here have any information or recommendations that could help?

    Homeowners Insurance, Hail Damage, Roof Replacement

    Hello...my wife and I bought a home (built 2001) approximately 7 months ago. Over the weekend we had some hail that in surrounding communities caused massive damage, but overall it seemed our area was spared. Yesterday some guys from a roofing company were driving around offering "free roof inspections" due to the hail. Obviously they are just drumming up business and I'm trying to be very wary of a potential scam, but allowed them to climb my roof. Afterward the guy showed several pictures that do show damage from hail and he claimed that there would be enough damage for my homeowners insurance to state total loss and replace the roof new. Of course everything would be done through my homeowners insurance and no work would be done without insurance approval...unless I plan to pay out of pocket.

    My question: the roof is original, so 18+ years old. At inspection during home buying process it was in decent condition with inspector stating at least 15+ years of life left without damage. Obviously roofs are expensive to replace, so I love the idea of having the roof replaced at a minimal cost to me (deductible), but is this is a smart move? I grew up living in hurricane country and know my parents filed two isolated claims two years in a row and the homeowners insurance threatened they were reviewing the policy for a potential cancelation (same company...we both use USAA). The cancelation didn't happen, but I've always been wary of using our homeowners insurance, but also never had a situation such where it would have been needed. Any input would be greatly appreciated.

    Allstate Extended Care?

    So big question here. I am currently buying a used new car 2019 with 18,000 miles on it. The dealership is asking for about 3,000 extended warranty. But I just saw Allstate's new extended insurance plan. It is only about 2 grand and lasts to 100,000 miles and even covers all dent and rock chip damage as well as worn tires. Seems too good to be true? Has anyone had any experience with this? The things it covers seems too good to be true and is in the link

    Need help with out of state insurance

    I live in NYC, and want to buy a motorcycle there. My license is from Michigan, and I don't exactly have a form of permanent residence (currently a student). My dad is in the process of moving from Michigan to California, so his car is currently insured in Michigan. If I buy a motorcycle in NYC, where do I get it registered, and how do I get it insured?

    Issue with Renter's Insurance?

    So I was recently robbed at gunpoint (about 2 months ago now), 2 intruders came in through a back window, I came home around 1:30 AM and there was 2 guns pointed at my face as soon as I opened my door, I was tied up, my car was stolen, all of my valuables, and a couple of guns.

    After the matter, about 2 weeks passed and I filed a renters insurance claim. I was told I need more evidence than just pictures (when I spoke to the insurance company and they pushed me into getting renters insurance, called for car insurance, I wasn't told anything about receipts). I only have a $5000 policy, although about $15,000+ was stolen worth of valuables. Problem is the only receipts I had were in my safe which were all taken out and stolen as well. What can I do?     Claim about car accident that we weren’t involved

    Hello, this is my first time posting it on this sub, but i got a letter today from my insurance company, about the accident two months ago that we weren't involved. My next door neighbor said that we backed into their car that's parked on the street, but the fact is we don't even drove that car for 3 months because my dad was out of the states. Tire was flat also. There was a minor scratch in the back but it was from the garage. Two policeman came and asked us questions two months ago, and they clearly said it's not our fault and we don't have to worry about it. However, after that, I think my neighbor claimed to insurance company (idk how) I tried to contact the claim associate few times, but he don't respond to my call or email so i'm kinda annoyed. Anyone know what can I do?

    Car hit in driveway, advice

    Yesterday evening my wife's new Subaru (2019 with 5K miles) got hit while parked and unoccupied in our driveway. The damage seems fairly extensive, all panels and doors dented and scraped on passenger side , bent wheel/popped tire, damaged bumper. The car that hit her was driving on a rim before the accident (I was out side and saw the accident). The lady driving was uninjured and we had her sit in a lawn chair until the police arrived. The driver was arrested for OUI drugs.

    We have contacted our insurance and they are going to send an adjuster today. Any advice on how to handle this situation? Both cars are insured. We were told the driver had Geico. Thanks.

    How long does it take for insurance company to process a car total loss claim? CT

Since it has been three weeks since the accident, no updates so far except for the "maybe totaled" info. I am worried. How long does it take typically for this kind of car total claim?     Woman dented my car, she gave her insurance info, but AAA says she's not answering phone

    Guys, this older lady dented my car slightly and the paint also got peeled a bit as she was backing out of the parking spot to the right. I was in the car at the time. She first denied she hit anything, then after I showed her paint of my car on the long scratch on her car, she was apologetic. She gave me her license, insurance info, etc. I called my insurer and they said if I go via my insurer they can cover me under comprehensive, but I have to pay deductible. But, since it's not my fault, they said to contact the lady's insurer (AAA in this case) and they should cover me completely.

    Called them and filed report and a AAA agent called me back to get my statement. But she said that the woman was unreachable and they will keep trying for 30 days to get her statement. If not reachable for 30 days, they will deny claim unless I have video evidence. I have photo evidence that shows scratch and my car's paint on her car that is consistent with what happened. AAA lady said this is just how all insurers work. Seems unfair that all that's required in this case is for the at-fault side to not just answer phone. I didn't call cops or file report since it seemed minor and figured she'd do the right thing.. What would you suggest the next steps should be? Thx.

    Totaled Car Coverage Question

    Hello! My sister was recently in a car accident in which she side swiped 2 cars and totaled her own. Thus far, she has not received any claims on her insurance from the people whose cars she hit and we're unsure if any will ever come through.

    Given the car was totaled, her plan was to remove it from the insurance and replace it with coverage for her new car.

    My question is this - can she actually remove the old car from being insured if there is still a possibility for claims to be made against it?

    Tips for interviewing as an experienced adjuster?

    I started as an entry-level WC indemnity adjuster a year ago at a large TPA and want to jump ship to a firm that better suits my career goals. After a phone interview today with one such firm, I have an interview Friday morning.

    It's a mid-level position. Prepping for the entry-level role last year was a snap thanks to an industry friend who started as an adjuster, but this feels different. Experienced adjusters, what should I expect for this interview? What's the best way to prepare?

    Underinsured Payout Concern - Fatal

    State: AZ So my brother (single/no kids/no will) was in a fatal accident in March while riding a motorcycle that was not his. The lady (78yo - 1995 Toyota) who hit him has state minimum coverage (15/30/10). Her insurance company is claiming 70% responsibility due to my brothers speed. The owner of the motorcycle has been compensated for the loss of property without underinsured coverage being used. My brother had 25k in underinsured coverage on his motorcycle and 100k on his truck policy. The owner of the motorcycle has another 100k in underinsured coverage.

    We are being contacted to address the claims and have a payout issued. My mother is a few weeks from death and my father (semi-estranged) has 400k in medical debt. My parents are legally married, but have been separated for 26 years. We will wait until my mother passes before addressing this, and my father wants to waive his rights and have the siblings (3) recieve all payouts. My brothers insurance company advises this is possible with proper documents, the bikes insurance is 'looking into it'.

    I assume this is unusual but possible. We will seek legal advice, and consider a lawsuit but I doubt its worth going after anyone here. Can anyone confirm that this is generally a possible request? Can you go after the insurance companies (the lady and/or motorcycle owner) for anything outside of the coverage? A civil claim against the lady is probably fruitless, but probably worth more than the 15k we stand to see for his death minus medical bills (sub 5k) from the evening. A claim against the motorcycle owner is unwise as we have ties to them and wouldnt want to damage that relationship. I understand an additional claim to the insurance could cause further increases in rates for him and have a similar, but less personally morivated impact.

    Personally, I'd just take the 225k and walk away, it was a true accident, no one was intoxicated. It's also his siblings and not a wife/child who rely on him to provide. I also dont want to deal with the additional headaches knowing I'm going to be cleaning up some of my mothers underprepared affairs as the executor. That said, I know my siblings and father will at least want to explore their options. With these payouts... do they go to his estate, or is it simply a benefitiary payout out?

    Any thoughts, concerns, input etc. ?

    I've been a dedicated employee for a certain retail company for almost 7 years. I recently found a new retail job that pays better, and I'm in the process of trying to figure out what I need to do for my health insurance.

    I found my own separation packet (they were going to wait until my last day) 4 days before my last day, and it said "Your coverage ends at midnight on your last active day of employment."

    First of all, is that normal? I've heard most companies give you a grace period. I could be wrong.

    Second, because of how this fell on the calendar I cannot get healthcare coverage until July from the Healthcare Marketplace.

    I have Cystinuria. It's when your body forms an endless supply of sharp kidney stones which leads to very expensive ER visits. One kidney stone without health insurance means I'll be at least 10k in debt.

    I've talked to the benefits department and HR. Both are sticking to their policies. I've worked through blood, sweat, and tears for this company and now I feel very under appreciated and royally screwed over.

    How can I fight this?

    Is an IUD placement check after 1 year preventative care?

    My IUD felt out of place recently and I went to have it checked by my PCP, who referred me to get an ultrasound to check that it was properly placed. I assumed this was preventative care, as it was related to birth control, which under ACA is considered preventative (I'm in the US). I received an huge bill (~$1500) for both the check-up and ultrasound and I'm not sure how to proceed. I just called my PCP's office and they told me that outside of the normal ~6mo-1yr window it isn't considered a routine check of placement, and thus isn't preventative. Is this correct and what I received wasn't preventative care?

    My car was stole and involved in a hit and run. The auto insurance also classified the case as theft. Why would I still have to pay the deductible if it wasn’t my fault?

    (Not at fault accident) GEICO vs Body Shop

    Hello friends,

    Recently, my car (parked) was hit my a driver that has USAA. USAA did not pick up their phone as the accident took place during the weekend, so I decided to fix the car with my insurance (GEICO) first, then make USAA reimburse money (subrogation).

    Anyway, the local body shop that has my car told me that my insurance(GEICO) is only going to pay for a $97 front bumper, when the OEM bumper is $400. The mechanic told me I can either pay the different to have the OEM bumper on or they can try to put the aftermarket bumper, but they cannot guarantee any fit and quality.

    My questions are,

    1. GEICO is going to pursue after USAA trying to get the cost reimbursed. Why does my own insurance company, that I've been insured for 10 years, have to try to low-ball do me like this? They can see that I have OEM bumper at this time, why wouldn't they just replace it with the exact same part?
    2. I called GEICO claims and they told me to talk to the auto damage adjuster who wrote the estimate. I am going to call him first thing tomorrow morning, but is there any way for me to make GEICO pay full price for the bumper? Should I reach out to USAA?

    tl;dr:

    1. not at fault accident happened. I have GEICO. the other has USAA.
    2. Decided to repair under my insurance first (GEICO) and GEICO wants to use $97 aftermarket bumper (OEM $400).
    3. I have to pay the difference if I want OEM, but why is GEICO doing this to me when they are going to charge USAA anyway?
    4. how do I make GEICO pay for OEM bumper? should I talk to USAA?

    Vehicle plated in one state, insured and driven in another.

    I live in North Carolina, but the vehicle I drive is owned by my mother who lives in Indiana. I know that you need to insure the vehicle where it is located, but is it okay that the vehicle is registered in another state? Do I need to tell my insurance company? When getting quotes, they never ask for this information.

    Alberta Insurance Insanity

    The low-down...my wife and I have 40+ yrs experience driving. No claims ever. Maybe a speeding ticket once every 2-3 years. Our insurance guy is telling up we are on the hook for a huge increase in rates due to gov't regulations. The more I dig the more it sounds like companies doing things to skirt the new rules and guarantee their income and commissions. Is there no competition anymore?

    Regular rates and payment schedules aside: Can a $100 speeding ticket seriously turn into a $600/yr for 3 year hike in insurance rates? That seems beyond insane!

    Does anyone know where these changes and ramifications are outlined online???

    In-Network Doctor used Out-Of-Network Lab?!?

    Do I need to disclose that I've caused an accident with my friend's car to my new insurance company? (TX)

    Hi. About a year ago I was in a very minor accident (my fault) while driving my friend's car. He had insurance on this car and so did I on my car. A cop responded but I was not given a citation. My friend provided his insurance information and the person I hit gave him theirs. My friends insurance called him around an hour later saying the victim was filling a claim against my friends insurance, but my insurance didn't say a peep to me.

    As far as I know, my insurance doesn't know about the accident. I'm currently looking at switching insurance providers, but they are all asking me to give an accident and ticket history. Besides this accident, I have a clean record for this period of time. Do I need to tell them I was involved in an accident if it wasn't my car? Is it even on my record? Thank you for your advice
