Posted: 27 Feb 2019 09:37 PM PST Hey all. I wasn't sure where or who to ask about this, so I am hoping I can get some answers here. I got a bill in the mail for a miscarriage procedure from a local OBGYN's office I am not a patient at and have never been to. I have never had a miscarriage, never went to the ER or anything of the sort. But my bill said I had this procedure done on Jan 21, filed to my insurance which paid about $600, and I now owe about $500 out of pocket. I called the practice first and they said there was no record of me having a procedure that day (or record of me ever being there) but I am in their system because they are under a large hospital chain and I have been to an urgent care facility owned by the same company. That makes sense, but not on how my ins got billed. So I called the chain's billing number that was included on my bill. They said they would reverse the claim but had no answers for me either and to call back in 2-3wks (sounds like a nightmare waiting to happen). So next I called my ins company (blue cross blue shield) and explained the issue to them. They were concerned about getting the claim reversed, but didn't seem that concerned about how it happened in the first place. So, /r/Insurance, I'm here to ask what I should do next and hopefully get some answers on what happened? It makes no sense to me how I could be billed for a procedure at a practice I'm not even a patient of, better yet wasn't even there the same day for something to get mixed up. I have thought about calling the practice again and asking them to check Jan 21 and see if someone did have those procedures done that day and to see if they can find out what went wrong (if anything) on their end. I don't need to know the details, I understand HIPAA, but I would like some reassurance on how this happened. She was driving her aunt's vehicle (our van had a flat tire) and she hit another vehicle from behind in the rain. Damage was minimal, both cars drove away. The police officer arrived at the scene and had a report written up which I'm sure indicated that the lady was fine and left the scene without anything broken or bleeding. My wife's aunt's insurance policy took care of both vehicles, we thought all was done. Now 3 months later the other lady is using a lawyer to claim for medical records above and beyond what she already got, asking for $30k to resolve the claim. We received notice from my wife's aunt's insurance that the lady's lawyer sent the demand for "limited bodily injury release" and they mailed us a copy of this legal demand from the lawyer. Her aunt's insurance is asking us if we have any additional car insurance or umbrella insurance. We have not replied yet. What should we do in this situation? We only received notices via regular postal mail, about 3-4 letter's thus far. Nothing has come via certified or courier that requires signature and they don't have her phone number. Should we ignore the letters until they send something more serious? We are trying to avoid our insurance getting involved in case our rates get affected or what if the aunt's insurance tries to stick us with the bills and then our insurance refuses to pay. Thoughts on this disability insurance policy? Is it worth it? Posted: 27 Feb 2019 06:03 PM PST Total yearly cost would be about $1700 with coverage for total disability after 90 day elimination period (and your own occupation coverage) and also has a rider for residual disability rider for losing 20% of your income d/t injury/sickness AND unable to perform some duties or work full time in your own occupation (guaranteed at least 50% of your monthly benefit for residual disability). Monthly benefit would be $5610 (67% of my monthly income) in the event of total disability and could continue until age 65 Disability phrased as "you must be unable to perform the substantial and material duties of your occupation and you are not working" Of note, pregnancy and childbirth are covered with after the normal 90 day elimination period. We have not yet had children but will be starting to try within the next 6 months. We are currently renting an apartment but will be buying a house in the next 4-6 months and want to protect our income. My husband is also working and has disability insurance through work that he does not pay for. Together we make around $200,000 before taxes each year. We also ran the numbers for a monthly benefit of $4000 rather than $5610 and this cost would be about $1200 per year. Policy would be through pharmacists mutual (via principal) Thoughts? It increased to about $70 following a speeding ticket, and then later increased to about $80 when replacing an older vehicle with a newer vehicle. With the latest policy renewal it went up to $120. Root claimed it was based on account of their premiums being too low in the first place as well as having to comply with state insurance boards. I've never actually heard of this. I'm curious how much longer Root can remain as competitive as they have been. [link] [comments] |
Express scripts Co pay override help Posted: 27 Feb 2019 01:50 PM PST I'll try to keep it brief. I am on a name brand drug that offers a generic. I do not tolerate the generic, it causes debilitating side effects. My previous employer offered coverage through EXs and I paid less than $60 per month for NB. New employer, new plan. At first I was paying out of pocket for Rxs with an HSA. Then the manufacturer raised prices from $180 to $330. I was stretching to afford 180. I chose a new, more expensive, plan this year with "better" Rx coverage. So now my copay for name brand is $313, because there is a generic available. ( I did go over my policy before I chose it, this was buried somewhere in the very fine print in an index) I'm paying $50 more a month for that 18$ savings. I have tried to call the manufacturer to see if they offer a discount(they don't), I have tried Goodrx and another recommended sight and none of them offer more of a discount than the $313. I absolutely cannot afford that every month. I called EXS for what feels like the 15th time and someone finally tells me I can ask for a copay tier override. Originally they told me this was something my Dr would have to do. Then they told me my plan does not offer Dr overrides and I needed to write a letter myself. Can anyone with knowledge of a carriers inner workings give me some tips for what to say or how to say it, so that my request gets approved? [link] [comments] |
CCC Valuation seems to have a mistake. Posted: 27 Feb 2019 09:15 PM PST I had a total loss and my car was valuated by CCC. The following link shows that there seems to be an issue. Can anyone help explain whats going on here? In conclusion, based on the Comps listed and the information and pricing structure laid out in the document provided, the vehicle value should be higher than what is portrayed. Without accounting for the options errors that were done, only taking the "condition" error into account, the new mean value that should have been offered is $26,084.62 as opposed to the original valuation of $24069. Also on another note, can someone explain why they are adding a vehicular tax of 6.5% (local sales tax) to my value. Is there some way of me recouping this money? (Illinois) Posted: 27 Feb 2019 09:31 AM PST My insurance went from just over 1000 to over 1600/year after I got a ticket for improper use of a turn signal. No other tickets, and this ticket doesn't result in the loss of any points. I checked with another company and they also had a drastically higher rate because of this. My daughter requires hearing aids, and they are not covered by my health care plan. My plan is also considered "grandfathered" by the Affordable Care Act. Posted: 27 Feb 2019 07:45 AM PST This is a really convoluted situation, so I'm going to try to make it as clear as possible. All personal information is made up for the purposes of this post. If I need to clarify anything, please just let me know. I had been with a national insurance company since 2003. Never had any problems until the last few months. Late last year, my husband and I bought a house. I called the day of closing and updated my address, dropped our renter's insurance policy, and asked for quotes on our auto insurance coverage. I was told it would drop to $150 and our December payment would only be $100 due to dropping renter's insurance with the processed refund. Great! Our December payment was processed as quoted. But, we began receiving policy information for someone else at our new address.
While my husband and I do have a somewhat common last name, it is not as common as Smith. I call 1-800-national company, and they re-route me to our local office. They pull up everyone's information and confirm addresses.
So, we share same last name, same house number (where my husband and I moved FROM), and same town (where my husband and I moved FROM). So, cool. Someone made a simple mistake, but everything is straightened out and we're good to go. Fast forward to the end of January, and premium payment is draft from our bank account. It is $230. Local office explains that the lower premium I was quoted was never accurate, and never should have been said to me. She then says that our policies weren't underwritten until January, which is why our premium was higher for January? Our actual premium was going to be $185. Honestly, nothing that she said was making sense to me as far as the refund we received on our December payment and now the higher charge for January's payment. She then says "you should have received something in the mail explaining all of this." Which...I had not.
This is incorrect. This is the address of Julie Smith. At this point, I'm done. This is multiple instances of not being able to get our addresses correct. The premium quotes have changed dramatically. I can't get a good explanation of why we are being charged what we are. Later that day, I switched our auto insurance to the same carrier as our homeowners, effective Feb 1. This should have solved all of my problems. This brings us to today. I received a piece of mail regarding a new policy to:
I don't know who Dave is, but it's not my husband. And why my name and current address is now attached to a new policy for someone else, I've got nothing. (Yes, I monitor both my credit and my husband's credit so I do not believe this is fraud or stolen identity). How do I get this resolved? The 1-800-national-company tells me I need to deal with the local office. The local office is clearly unable to handle this situation as it needs to be. Is this something I can reach out to the insurance commission about? Is there some sort of magic word or phrase to end this once and for all? Can someone provide some insight into what is actually happening on their end so I can be less frustrated? EDIT: And now that I'm thinking about it...the refund check that we received for Feb's premium's was for the amount I was quoted in December for $150, not the amount I was charged in January of $185. No visible damage to his bumper, didn't even check my bumper as I drive an old beater car... I haven't been in a traffic accident in over a decade, so I have no idea what to do here. He said he would contact his insurance company and get back to me. The insurance card I had on me was expired. He refused to take any further information, not my name, not my policy number... He wasn't rude by any means, I'd be annoyed if someone backed into my car as well. I took photos of his bumper after he went inside... Ontario Garage Policy OAP4 help Posted: 27 Feb 2019 03:04 PM PST Hi, I deal primarily with the OAP1 in my current job. I have been working with a repair shop with respect to damage to a car they rented to our insured. The damage occurred in Ontario, our insured is 0% at fault and the at fault party is insured with an Ontario signatory insurer. As such we have denied coverage through the insured's policy as the vehicle has DCPD coverage on it through the owner's policy. The shop is pushing back stating that the OAP4 does not cover this. In reviewing the OAP 4 there are exclusions for DCPD losses that state : THE INSURER SHALL NOT BE LIABLE while, (a) the automobile is rented or leased by the Insured to another, provided that the following shall not be deemed to be renting or leasing to another, (ii) the use of an owned automobile by a customer pending return of the customer's automobile which has been left with the Insured for repairs or servicing When I read that it sounds like the OAP4 will also not cover this damage and the garage as the owner of the rental vehicle will be responsible. Benefits/Negatives of a Hospital Income Policy? Posted: 27 Feb 2019 01:37 PM PST 19 year old female, a few months ago I registered and insured my vehicle from my mothers name and into mine. I chose a local State Farm agent since SF is a pretty reputable company. This agent had me sign a hospital income policy which would pay me ~$300 for emergency only hospital visits. It sounded fine enough to me as she told me it would save me 15% on auto. Now 2 months later I received a suspension order in the mail from the DMV stating that SF had denied coverage of my vehicle (on a policy that I have been paying for). It was an agent error that she admitted to me she had processed wrong and has since corrected. Regardless, it was a really scary position to be in that should have never happened and I just want to cut my losses with her. I visited another local SF agency to talk about transferring to there. They were really confused as to why I had signed the hospital income policy, basically said there was no reason for a client of my demographic (age, gender, education) to have signed this legal document that in reality only saved 5%, not the 15% I had originally been told. (They also pointed out a lot of other things that were added to my policy and I didn't need making me feel like I had really been taken advantage of, being new to the insurance game.) This agent told me it was my decision whether or not to keep the hospital income policy or terminate it when I transferred to their location in a few days. Property insurance on business condo (South Carolina) Posted: 27 Feb 2019 06:55 AM PST I own one of five units in a commercial building that is classified as a live/work space. There are four other owners, and three of them rent their units out to either businesses or a residential tenant. We pay POA dues to a larger association that covers the entire business park, but property insurance has always been paid as a 1/5 share of a policy that covers just our 5-unit building. The owner who handled this payment every year has recently sold his unit and just gave us a copy of the policy which is due to be paid soon. Reading the policy, we've learned that:
Another issue is that some of the owners are fairly unresponsive, basically it's just two of us trying to make sure we still have coverage and chase down the checks from the other three. Because of all these issues, we'd rather not pay the inflated premium for this policy, and would instead like to get a new policy that just covers the property itself and let business owners/residents get their own policies for personal/business liability. My questions are:
